Long Term Care

The assistance required by an individual who has a chronic illness or disability that leaves him or her unable to care for him- or herself for an extended period of time. Long term care may be provided in a residential care facility (e.g., an assisted living facility, a skilled nursing facility), an adult day care facility, or the individual’s own home. Such care includes both skilled nursing care and custodial care, and may be required by individuals of any age.
